Registration Information and Forms

Isle Return Program

Enrollment forms are available at the Elder Services of Cape Cod and the Islands office, Nantucket County Sheriff’s Office, Nantucket Police Department, Nantucket Fire Department, Nantucket Cottage Hospital, Saltmarsh Center and local physician’s offices. Completed forms may be returned to any of the same agencies. The information will then become part of a confidential database. Unless required sooner, updates of the information will be performed every six months.

911 Disability Indicator

The disability indicator program is voluntary for both the community and its residents and was created by a group of several different organizations representing the mobility, hearing, speech and sight impaired communities.

The information provided on the disability indicator form enables a special code to appear on the 911 call takers screen which alerts the call taker that a person residing at that address may require special assistance during an emergency.
